Adult Supported Living

For our clients aged 18 and over, we provide different packages of support aimed at promoting recovery and independence. In our supported living provisions we provide accommodation and support for people with enduring mental health difficulties that have been selected by commissioners to continue their recovery journey in the community with the ultimate aim of eventually helping them secure their own tenancies with minimal support where possible. We also work with people who are under the Mental Health Act including Ministry of Justice (MOJ) sections who are also supported on DoLS or conditional discharge. Our provisions also cater for those who have co-morbid Mental Health and Learning Difficulties.

For Care Co-ordinators, we have simplified our information sharing protocol by giving them password protected access to our information recording system at any time of their choosing. The system is cutting edge and will give them access only to service users under their care. They will have real time access to Notes, Care Plans, Appointments, Risk Assessments etc. This makes multiagency working easier and interactive.

 

For our service users we offer the following in our supported living provisions;

  • 24 hour staff presence
  • Recovery and outcome based support
  • Ensuring they take any prescribed medication by reminding them regularly
  • One to one or two to one support as appropriate
  • Therapeutic intervention by selected providers
  • Supported living for people with autism
  • Supported living for people with mental health difficulties
  • Pathways to their own tenancies where appropriate
  • Life skills training
  • Social and community integration
  • Minimal staff turnover
  • A service user mix that is sensitive to gender, age and risk profile
